Terms and Conditions for No Gap Comprehensive Check Up
No gap for check-up and clean only for patients with health fund.
Key conditions: 1. Health fund should still have the balance available to cover for the treatment. 2. The HF should cover minimum of $110 for check-up and clean. If not -remaining gap payment would be payable by the patient.
If no health fund discounted price of $135 including check-up, clean, polishing and fluoride application.
Balance must be claimed/paid on the day of treatment/appointment.
